First Passport

This is the information on how to apply for a first U.S. passport. Please skip to the portion that best describes your situation to find specific instructions. Should you have additional questions, please contact us. If you have an emergency need for passport services outside of these hours, please contact us to set up a special appointment. We cannot issue passports outside of working hours.

Adults

  • Proof of U.S. citizenship (e.g. U.S. birth certificate, naturalization certificate);
  • Proof of Identity (e.g. driver’s license, ID card);
  • Completed application form DS-11 (PDF 102 KB);
  • One photograph, taken within the last six months, 2″x 2″ with dimensions as shown on the application form;
  • $145, payable in U.S. dollars, Serbian dinars, or with major credit cards.

You must apply in person at the Embassy. Make an appointment to submit your passport application.

Minors (age 16 and 17)

Minors (under age 16)

  • Proof of U.S. citizenship (e.g. U.S. birth certificate, consular report of birth abroad, naturalization certificate);
  • Proof of Identity (e.g. driver’s license, ID card);
  • Completed application form DS-11 (PDF 102 KB);
  • Evidence of parental relationship (birth certificate or adoption decree.  Foreign documents must include a certified translation if not written in English, unless they are on the international form);
  • One photograph, taken within the last six months, 2″x 2″ with dimensions as shown on the application form;
  • $115, payable in U.S. dollars, Serbian dinars, or with major credit cards.
  • Applying parent’s photo identification (e.g. passport, driver’s license, ID card)

The minor and both parents must appear in person. If one parent cannot appear, the non-applying parent must submit a notarized DS-3053 (PDF 51.5 KB) consent form. If a written consent of the non-applying parent cannot be obtained, DS-5525 (PDF 42.6 KB) “Statement of Exigent/Special Family Circumstances”, must be submitted.
