Name Changes, Error Corrections, Extensions of Limited Passports

We cannot amend valid passports.  U.S. citizens requesting name changes, replacement of emergency passports, or correction of a printing error must apply for replacement passports. This page will give you information on how to replace your U.S. passport. Name Changes (within one year of issuance of passport)

  • U.S. passport that needs the change;
  • Completed DS-5504 (PDF 89.3 KB) form;
  • One passport photo, taken within the last six months, 2″x 2″ with dimensions as shown on the application form;
  • Documentation showing the name change (marriage certificate, divorce decree, court order, etc.). Please note that all documents must be on the international form, or include a certified translation if not written in English.

Make an appointment to submit your passport application. All payments are made at the Consular section of the U.S. Embassy at the time of your appointment.

Name Changes for Adults (more than one year after issuance of passport)

  • U.S. passport that needs the change;
  • Completed application form DS-82 (PDF 88.5 KB);
  • One passport photo, taken within the last six months, 2″x 2″ with dimensions as shown on the application form;
  • Documentation showing the name change (marriage certificate, divorce decree, court order, etc.). Please note that all documents must be on the international form, or include a certified translation if not written in English.
  • $110, payable in U.S. dollars, Serbian dinars, or with major credit cards.

Make an appointment to submit your passport application. All payments are made at the Consular section of the U.S. Embassy at the time of your appointment.

Name Changes for Minors (age 16 and 17) more than one year after issuance of passport

  • Proof of citizenship and identity (your U.S. passport);
  • Evidence of parental relationship (birth certificate or adoption decree.  Foreign documents must include a certified translation if not written in English, unless they are on the international form);
  • Completed application form DS-11 (PDF 101KB);
  • One photograph, taken within the last six months, 2″x 2″ with dimensions as shown on the application form;
  • $145, payable in U.S. dollars, Serbian dinars, or with major credit cards.  All payments are made at the Consular section of the U.S. Embassy at the time of your appointment;
  • The minor and one parent must appear in person;
  • Applying parent’s photo identification (e.g. passport, driver’s license)
  • Documentation showing the name change (e.g. court order, etc.). Please note that all documents must be on the international form, or include a certified translation if not written in English.

Name Changes for Minors (under age 16) more than one year after issuance of passport

If a written consent of the non-applying parent cannot be obtained, then DS-5525 (PDF 47.3KB) “Statement of Exigent/Special Family Circumstances”, must be submitted;
  • Applying parent’s photo identification (e.g. passport, driver’s license)
  • Documentation showing the name change (e.g. court order, etc.). Please note that all documents must be on the international form, or include a certified translation if not written in English.

Error Correction (information in passport printed incorrectly)

  • U.S. passport that needs the change;
  • Completed DS-5504 (PDF 89.3 KB) form;
  • One passport photo, taken within the last six months, 2″x 2″ with dimensions as shown on the application form;
  • Documentation showing the correct information (prior U.S. passport, birth certificate, naturalization certificate, etc.). Please note that all documents must be on the international form, or include a certified translation if not written in English.

Replacement of Emergency Passports (within one year of issuance of passport)

  • U.S. passport that needs the change;
  • Completed DS-5504 (PDF 89.3 KB) form;
  • One passport photo, taken within the last six months, 2″x 2″ with dimensions as shown on the application form;
  • Letter given to you at the time of issuance of your emergency passport.
